Please Note: This service if for housebound patients within SMG boundary

Home visits can be arranged for patients who are housebound because of illness or disability and live within our practice boundary, please try to telephone reception before 10am if you require a home visit.

A clinical practitioner may phone you back as it may be that your problem can be dealt with by telephone advice.

Please remember that several patients can be seen in the practice in the time that it takes to make one home visit. There are also better facilities for examining and treating patients at our Surgeries.

If you are an out of area patient and may require home visits we recommend you register with a practice closer to home to facilitate this service.

Housebound Status – What You Need to Know

At Swan Medical Group, we aim to provide the best care for all our patients. Understanding the definition of housebound status helps us ensure that care is delivered in the most appropriate way.

What is Housebound Status?

A patient is considered housebound when they are unable to leave their home environment due to a physical or psychological illness. This means they cannot leave their home without significant assistance or the use of specialist transport, such as an ambulance or a vehicle requiring a two-person escort.

Housebound status is reviewed regularly, as we recognize that a patient’s needs may change over time. Some patients may only require this classification temporarily, depending on their circumstances.

What Does Being Housebound Mean for Your Care?

If you are classified as housebound, we will provide routine clinical appointments in your home. These may include:

  • Blood pressure checks
  • Vaccinations
  • Asthma reviews
  • Other essential care

This ensures you continue to receive the medical support you need without needing to visit the practice.

Temporary Housebound Status

In some cases, a patient may be classified as housebound on a temporary basis due to a short-term illness or condition. During this period, home visits will be arranged as necessary, and the classification will be reassessed regularly.

Not Housebound?

If you are able to leave your home with minimal assistance, such as traveling by car, taxi, public transport, or with an escort, we kindly ask that you attend clinic appointments. Clinics provide a wider range of facilities and resources to support your healthcare needs.

Examples of activities indicating a person is not housebound include:

  • Visiting the doctor, dentist, or hairdresser
  • Attending social events or hospital outpatient appointments
  • Going to the supermarket or similar activities
