Swan Medical Group
The Swan Medical Group is a Partnership and a Primary Care Network, comprising of five surgeries across Petersfield, Liphook and Liss. Rated as Outstanding in our most recent CQC inspection, we pride ourselves in offering the highest quality of healthcare through a personal and dignified service for you and your family.
We invest time in getting to know our patients and being able to provide continuity of care, every patient has a named GP, this way we can take a more holistic approach to our patient’s health and wellbeing. We are Veteran, Dementia and Learning Disability Friendly, as well as having Autism Ambassadors as part of our fantastic team.
We have suitable access for disabled patients, and all the patient areas including the waiting rooms, consulting rooms and toilets have wheelchair access.
The clinical team comprises not only of Doctors, Advanced Nurse Practitioners, Practice Nurses, Paramedics and Health Care Assistants, but also Pharmacists, Social Prescribers, Physiotherapists and Pharmacy Technicians. They are supported by teams of Care Co-ordinators, Receptionists, Medical Secretaries, Administrators and our Middle & Senior Management teams.
